Midwives are in a precarious position in China as they must juggle caring for tens of thousands of children in homes, nursing elderly relatives and making money for families.
It is a job that many Chinese expect to disappear in the coming decades.
The country’s government recently said it would require midwives to get more money from their clients.
A survey released by the Beijing-based Center for Social Reform last month found the median income for midwives was only $11,000.
The survey of midwives also found that while they earned more than 50% of their income from their patients, they were not paid enough for their work.
The center’s report found that half of all midwives said they received less than 20% of the money they earned from patients.
The survey also found midwives earn less than 50 cents for every dollar they earn from patients, while many midwives receive less than 5% of income from clients.
It said a midwife’s average salary is only $6,800 a year, while a teacher in Beijing makes around $30,000 a year.
The study also found half of midwife respondents said they earn less per patient than doctors.
“In China, midwives are still in a tough position, with little freedom to choose their profession, and their income depends on their performance,” the center said in a statement.
“The government is trying to push midwives towards becoming doctors, which would make their salaries more stable.”

But midwives working as nurse midwives earned less than $20,000 annually, and some were making more than half of their salary.
Midwives in the United States earn about $20 per hour.
Some midwives in China earn as little as $5 an hour, the survey said.
China has a large population of migrant workers from the United Kingdom and other countries, and they often are not compensated fairly.
Many are forced to work in poor conditions, while others are paid less than minimum wage.
The study said many midwifery jobs require a nurse midwifer to be certified, and nurses must have training to be able to do the job.
“The fact that nurses are the most common kind of midwitches in China is also due to the poor education level of many of them,” said Li Yang, the institute’s president and a midwister for five years.
“They are very young, and very inexperienced, and not trained to handle these conditions.”
The report also said midwives have been paid more than they are worth because of their high turnover rate.
“Many midwives can only earn about 10% of what they earned in their previous jobs,” Li said.
“A lot of them earn even less than their previous salary.”
